Application Process

Application Process and Requirements

Applications and additional forms can be obtained online at travel.state.gov or from the Marion County Clerk of Court Passport Department. Forms with any errors or corrected information cannot be accepted for processing. Passport services are available at the Marion County Clerk of Court Annex Building by appointment, including passport photos. Appointments can be made online using our Online Appointment Scheduler.

Appointments are necessary to avoid long wait times and to better serve you. Walk-in services will be available only AFTER scheduled appointments are completed. The latest appointment is 4:00 PM daily.

Applying for a Passport

Complete the DS-11 application form.  All applicants must appear in person.  The following documentation is required when submitting your application:

Renewing a Passport

Your passport was issued for ten years. You are allowed to renew it within a five-year period after it expires. If your passport was issued within the past 15 years, and you were at least 16 years when you received it, complete the DS-82 renewal application form. These applications are submitted by mail, along with the following documentation:

  • Your most recently issued U.S. Passport Book and/or Card
  • One Recent Passport Photograph
  • Proof of Name Change (if applicable, certified copies required)
  • Passport Fees

For current amounts, please see the Fee Schedule and Service Charges document or visit travel.state.gov. You must mail this form to the Passport Agency at the address stated on the form. We generally suggest sending this certified mail. The renewal will be issued for a ten-year period.

Minors

All minors must appear in person. Passports for minors are issued for a five-year period of time. They may not be renewed, a new passport must be issued by submitting the DS-11 application form. In addition to the regular materials required for adults, evidence of the child’s relationship to parents or guardian(s) and parental identification will also be required.

If only one parent appears for applicants under age 16, you must also submit one of the following:

  • Statement of Consent, DS-3053 Form, completed by absent parent and notarized within last 90 days.
  • Evidence of sole authority to apply (certified court order granting sole custody or a birth certificate listing one parent)
  • Statement of Exigent/Special Family Circumstances. DS-5525 Form, explaining why absent parent cannot be reached.
  • Absent parent must also submit a copy of photo identification, both front and back.
