Change of Name (Minor Children)

This packet of forms should be used when parents want the court to change the name of their minor chid(ren). For the purposes of this proceeding, a person under the age of 18 is a minor. This form is not to be used in connection with an adoption, dissolution of marriage, or paternity action. Note: Pursuant to §68.07 Florida Statutes, before a hearing is held on a Petition for Name Change, each adult Petitioner must have fingerprints submitted for a state and national criminal history check and the Clerk must have the results. You can go to the Clerk’s Office to obtain information on the process for having fingerprints taken and submitted, including information on law enforcement agencies or service providers authorized to submit fingerprints electronically to the Department of Law Enforcement.
