Jury Duty Excusal Form

Request for excusal must be submitted 5 working days prior to reporting or it will be denied.

If you choose an excusal reason that is not listed on this page, your request must be mailed in to be addressed by a judge. For example: I am physically sick or disabled (note from healthcare provider required); I am a practicing attorney or practicing physician; any other reason not listed.
